Hospital discharge


Often, patients will not be released from hospital unless there is 24-hour care in the home upon return.

We work with family as well as hospital staff and the local health authority to ensure that there is the proper level of care set up for the client, and that care is there for as long as required, for the hours that you require.

We can arrange a meeting in the hospital with the patient, family and nursing staff, and have our caregiver undergo the transfer of knowledge.

Our caregiver can temporarily reside in the home and provide for the care needs throughout the day and be there overnight as well, for as many days per week as needed, and as many weeks or months or years as required, and help with such things as:

Meal preparation • Cleaning/housekeeping • Personal care • Bathing • Incontinence care • Toileting • Mobility assistance • Shopping • Companionship • Medication reminders • General supervision • Dementia care • Overnight PEACE OF MIND
