Hospital discharge
We work with staff in the hospitals to arrange for a caregiver to be there upon a return home from hospital, to help with all care needs so that the client can continue to live comfortably at home. We can also offer a long-term solution through the live-in caregiver program that would offer the most consistent care for the best value. With our “bridge” caregiver to permanent live-in, we plan for the long-term, and manage the short-term.
